After making payment for the service, you will recieve a link to download the MPI data uptake sheets and the MPI photo guide. Be sure to include your name, address, contact information, and e-mail address you would like your MPI report to go.

Look over the uptake sheets and the photo guide to familiarize your self with the uptake process.

Look over all the areas of the home that are questioned in the MPI data uptake sheets to be sure you can answer the questions posed.

Circle the answers you feel best fit the questions

Transmit the results by filling out the form here, or mail the completed MPI data uptake sheets to

Within five days of receipt, we will send out by e-mail, a report of your risk score with opinions and an explanation, as well as specific recommendations of corrections that should be performed to minimize this risk.
